General Office Policies
Copayment is due at the time of service.
If appointments are not cancelled or rescheduled 24 hours prior to the visit, there is a $25.00 cancellation fee.
Unfortunately we cannot see a patient who does not have a valid referral. Please be aware that many primary care physicians are now requiring 3 - 5 days to process referrals. Referrals are your responsibility to obtain. If you are unsure as to whether you need a referral please contact your primary care physician.
